摘要解释偏向矫正训练(Cognitive Bias Modification-Interpretation)是一种可以使健康个体或社交焦虑个体解释偏向发生改变的训练方法,它通过不断重复一些简单的实验操作来改变个体对于模糊线索的解释。此训练主要在社交焦虑个体的解释偏向方面发挥作用,整个操作过程可以自己完成,不需要心理治疗师的直接参与。通过重复的不断练习,此训练可以“自下而上”改变个体的认知偏向。此外还可以把训练迁移至网络,让人们可以在家或在工作单位完成训练。本研究使用解释偏向矫正训练APP作为训练工具,对慢性胃炎、乙肝肝纤维化合并焦虑患者在传统治疗的基础上进行训练干预。通过前后状态-特质焦虑问卷的评估以及疾病疗效的对比,发现解释偏向矫正训练能够使状态焦虑和特质焦虑程度下降,进而改善疾病的治疗效果。研究 1选择河北省中医院门诊诊断和治疗的60例慢性胃炎合并焦虑患者,按随机数字表法随机分为解释偏向矫正训练干预组(A组)和对照组(B组)。A组除了常规疏肝和胃方治疗,同时使用解释偏向矫正训练APP进行训练;B组采用常规疏肝和胃方治疗。治疗前后分别用状态-特质焦虑问卷(STAI)评估患者焦虑水平,用尼莫地平法评估胃炎的程度和疗效。4周为1疗程,比较两组的焦虑水平变化和慢性胃炎的治疗效果。结果显示解释偏向矫正训练可减少患者的焦虑症状,降低患者的焦虑水平,进而改善慢性胃炎的治疗效果。研究 2 选择河北省中医院门诊诊断和治疗的60例乙肝肝纤维化合并焦虑患者,按随机数字表法随机分为解释偏向矫正训练干预组(A组)和对照组(B组)。A组除了常规使用抗病毒药配合疏肝健脾方治疗以外,还使用解释偏向矫正训练APP进行了训练;B组采用常规抗病毒药配合疏肝健脾方进行治疗。治疗前后分别用状态-特质焦虑问卷(STAI)评估患者焦虑水平,用临床证候评分、肝脏弹性测定评估肝纤维化的程度和疗效。4周为1疗程,比较两组的焦虑水平变化和乙肝肝纤维化治疗效果。结果显示解释偏向矫正训练可降低乙肝肝纤维化患者的焦虑程度,进而改善乙肝肝纤维化的治疗效果。通过以上两项研究发现,使用基于互联网的解释偏向矫正训练APP,可以使慢性胃炎、乙肝肝纤维化合并焦虑患者的状态焦虑和特质焦虑程度明显下降,缓解焦虑的同时明显改善疾病的治疗效果。
其他摘要Cognitive Bias Modification-Interpretation is a kind of means to change personal interpretation bias in healthy person or socially anxious person, changing subjects’ interpretation of fuzzy clues by simple repetitive experiments. The training directly affects interpretation bias of individuals with social anxiety. The training is not dependent on therapists, it can be done by ourselves. Personal cognitive bias can be changed from above through a lot of practices. It can also be migrated to the mobile internet as training aids, people can complete training at home or at work. This study used Cognitive Bias Modification-Interpretation APP as a training tool to intervene with anxiety levels in the patients with chronic gastritis or hepatitis B fibrosis with anxiety. By comparing the difference of anxiety level and therapeutic effect, we found that the level of state anxiety and trait anxiety can be reduced through the training,and thus to improve the therapeutic effect of diseases.Study 1 Sixty chronic gastritis with anxiety were divided into two groups randomly. Group A received shuganhewei Chinese medicine decoction therapy and Cognitive Bias Modification–Interpretation. Group B received shuganhewei Chinese medicine decoction therapy. State-Trait Anxiety Inventory(STAI) was used to evaluate the anxiety levels before and after treatment, and nimodiping method was used to evaluate the therapeutic effects of chronic gastritis. After four weeks of treatment, Changes in anxiety levels and therapeutic effects of chronic gastritis were evaluated. The result showed that Cognitive Bias Modification–Interpretation can alleviate the anxiety symptoms of chronic gastritis and improve the therapeutic effect of chronic gastritis.Study 2 Sixty patients with hepatitis B fibrosis and anxiety were randomly divided into two groups: the intervention group of interpretation bias correction training (group A) and the control group (group B). In group A, except for the treatment of conventional anti-virus drugs and Shuganjianpi prescription, the training was carried out with Cognitive Bias Modification-Interpretation APP; group B was treated with conventional anti-virus drugs and Shuganjianpi prescription. Before and after treatment, the anxiety level was assessed by STAI, and the degree and effect of hepatic fibrosis were evaluated by liver elasticity test and clinical syndrome score. Four weeks is a course of treatment. Changes in anxiety level and therapeutic effect were evaluated. The result showed that Cognitive Bias Modification-Interpretation can alleviate the anxiety symptoms of patients with hepatitis B fibrosis and improve the therapeutic effect.The above studies showed that the level of state anxiety and trait anxiety can be reduced by using Cognitive Bias Modification-Interpretation APP, the therapeutic effect of diseases was improved as anxiety was alleviated.
